Infertility is a problem affecting approximately 15% of expectant mothers and fathers. Male infertility is seen in approximately 40% of these couples. For prospective parents with infertility problems, the male should be the first to be examined because it is easier. There are many causes of male infertility. Sperm production disorders, blockages in sperm ducts, undescended testicle, presence of antibodies against sperm, radiation, alcohol, smoking, bilateral testicular absence, genetic disorder, mumps after puberty, liver and kidney diseases, testicular trauma, hormonal disorders, anatomical problems, previous diseases, infections and some medications can cause infertility. Whatever the cause, infertility is not easy to overcome. Many infertile people feel incomplete and unhappy. It is normal for people to feel this way and they can overcome this by communicating with other people. Infertile couples should support each other to overcome these problems. It should be remembered that 90% of infertility problems can be treated and there are many treatment options.          <\/p><p><strong>What should a man do if he has infertility problems?<\/strong><\/p><p>Prospective fathers with infertility problems should consult a urologist about IVF treatment. The doctor listens to the patient&#8217;s history and performs a physical examination. During the physical examination, the condition of the testicles, sperm ducts and whether the patient has varicocele are investigated. The patient will be asked for a spermiogram (sperm analysis) and other tests if necessary. The spermiogram evaluates sperm count, motility and shape. In the sperm test, fresh semen taken after 48 hours of sexual abstinence is evaluated. Since sperm count can vary, semen analysis should be performed at least 2 times. According to the results, the appropriate treatment method is determined for the patient.       <\/p><p><strong>What are the Symptoms of Azoospermia and How to recognize it?<\/strong><\/p><p>Azoospermia may not cause any symptoms until attempts to get pregnant have failed. If you have experienced symptoms before, they may be due to underlying causes such as hormone imbalances or genetic chromosomal disorders. But if he is experiencing symptoms, they may include decreased sexual desire, erectile dysfunction, glands, swelling and discomfort around the testicles, and loss of facial and body hair.To diagnose azoospermia, a spermiogram (sperm test) should be performed. However, you must not have had sex or masturbated for 3 days before taking this test. There is no need to extend this period to increase the sperm count. If the time is extended, the sperm count will increase but the sperm motility and quality will decrease. On the 4th day, the candidate goes to a center and ejaculates into a sterile container given to him and the sample taken is evaluated in the laboratory and the sperm count is reported.In addition to the spermiogram test, blood tests to assess hormone levels or genetic problems, ultrasound to see the scrotum and other parts of the reproductive organs, brain scans to look for problems with the hypothalamus or pituitary gland, and biopsies to better examine sperm production are also used.Along with the medical examination, the doctor will also look at the person&#8217;s health history. Fertility history, family history, childhood illnesses, various surgeries on the pelvic area or reproductive organs, history of infections, treatments such as radiation or chemotherapy, medication, drug or alcohol abuse, recent illnesses associated with fever, recent exposure to high temperatures are important information for diagnosis.<\/p><p><strong>What are the Causes of Azoospermia?<\/strong><\/p><p>In patients with azoospermia, sperm production occurs in the testicles, but this obstruction poses a problem because the sperm cannot be expelled with semen due to obstruction in the conduction ducts, or sperm production may be impaired in the body due to disorders in sperm production, i.e. congenital testicular elevation (undescended testicle &#8211; cryptorchidism), genetic disorders, hormone deficiencies, certain infections and radiation.<\/p><p><strong>How is azoospermia treated?<\/strong><\/p><p>For the treatment of azoospermia, the cause of the disease must first be identified. After the diagnostic tests are completed and the urologist&#8217;s examination, the treatment to be applied is determined.If there is azoospermia due to obstruction, the obstruction in the ducts is opened surgically to allow the sperm to come out and pregnancy is expected naturally after the ducts are opened. In azoospermia caused by a production disorder, the sperm required for IVF treatment is collected from the testicles by a surgical procedure called micro-TESE. During the micro-TESE procedure, an operating microscope is used to identify the ducts in the testicle that can produce sperm and samples are taken from these areas. Micro-TESE prevents damage to the testicle by taking small pieces of the testicle and increases the chance of finding sperm.Being diagnosed with azoospermia can make the patient anxious, but this does not mean that it is biologically impossible to have a baby. If azoospermia treatment does not yield positive results, in vitro fertilization can be used.<\/p><p><strong>What is Varicocele?<\/strong><\/p><p>Varicocele, one of the most common causes of male infertility, is the inability of the veins in the testicle, which carry the dirty blood coming out of the testicles, to fully fulfill their function due to enlargement and varicosity that may occur starting from adolescence.<br>Varicocele in the left testicle is seen in 90% of cases. It is thought that varicocele is more common in the left testicle because the left testicle is exposed to more pressure and a relatively slower blood flow than the right testicle. <\/p><p><strong>What are the Symptoms of Varicocele?<\/strong><\/p><p>Mild varicocele may not show any symptoms, but over a long period of time, as the enlargement of the veins progresses, swelling and pain in the testicles can be seen. When standing for a long time and in hot weather, the veins in the testicles become prominent and the pain increases. In very advanced cases of varicocele, the testicles decrease in size and soften in consistency. Varicocele is easily understood when examined by hand. It can also be detected by examining the vascular structure with ultrasound.    <\/p><p><strong>Does Varicocele Cause Infertility?<\/strong><\/p><p>The body temperature is several degrees higher than the temperature in the outer bags that protect the testicles. This means that sperm need a cooler environment than normal body temperature for healthy sperm production. The increased temperature due to blood pooling in the enlarged varicocele veins in the scrotum negatively affects sperm production and may impair sperm quality.  <\/p><p>In men with varicocele, toxic substances from the kidneys and adrenal glands can accumulate in the testicle. This can lead to a decrease in the movement and quality of sperm. <\/p><p><strong>Should Varicocele Always Be Treated?<\/strong><\/p><p>Not all cases of varicocele need treatment. If there are other causes that reduce sperm count, these should be identified and corrected first. The treatment of varicocele in cases of azoospermia, or \u201czero sperm\u201d, is controversial.
